RN Nurse Supervisor Position Summary:

The Adult Family Care (AFC) Program Nurse Supervisor is responsible for educating the RN staff on the initial medical assessment of the prospective program participants, as well as maintaining communication with client’s medical team. The Nursing Supervisor provides oversight of the implementation of home-based services. In addition, this person is responsible for identifying and accessing additional medical resources to support and to assist clients and their families in his/her caseload, and for maintaining direct communication with the AFC Program Director.

 

Supervisory Responsibilities:

  • Educates and completes the MDS-HC or CDS and other nursing assessments and coordinates all other applicable clinical assessments
  • Ensures that the PCP form for pre-authorization is completed annually
  • Reviews physician summaries and educates RNs
  • Reviews AFC caregiver logs monthly
  • Provides supervision to RNs at least twice a month including the reviews of client’s case files to ensure proper documentation has been completed and is up-to-date.
  • Other relevant job-related duties as needed including performance evaluations

Duties & Responsibilities:

  • Develops and reviews on an ongoing basis each member’s AFC plan of care in conjunction with the care manager, client and client’s caregivers.
  • Selects, trains, evaluates, and supervises AFC caregivers in conjunction with the Care Manager Supervisor
  • Monitors the health status of all members to ensure that all needed AFC care is properly delivered
  • Reports changes in the health status of any member to the member’s physician
  • Conducts on-site visits with each member at the qualified setting bi-monthly (alternating with the bimonthly visit by the care manager) for Level I, and monthly or more often as the member’s condition warrants for Level II
  • Completes nursing progress notes corresponding with each on-site visit or encounter, or more often as the member’s condition warrants
  • Submits a semi-annual health status report to the member’s primary care physician
  • Plans for and implements discharges from the AFC program
  • Conducts an orientation for each AFC caregiver before the AFC caregiver begins personal care
  • Provides ongoing training to AFC caregivers and RN staff
  • Mandated Reporter Statute: An individual in this position is considered a “Mandated Reporter” when on, or off duty according MA Statutes


Qualifications & Skills:

  • Must have strong interpersonal skills and be able to interact effectively both internally and externally with individuals from different social, cultural, racial and economic backgrounds
  • Ability to transmit, receive and accurately interpret ideas, information, needs and general business data through the application of appropriate communication behaviors
  • Strong communication skills both verbally and in writing
  • Ability to effectively present information and respond to internal/external inquiries (e.g. staff, clients, etc.)
  • Ability to work well under pressure in a fast-paced, time-sensitive environment
  • Ability to work on a multi-disciplinary team, as well as independently
  • Working knowledge of Microsoft Office products (Outlook, Excel, Word)
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists

 

Education & Experience:

  • Must be a fully licensed RN by the Massachusetts Board of Registration in Nursing.
  • BSN required, MSN preferred
  • Must have at least two years’ recent nursing experience in the direct care of the elderly or disabled adults.
  • Minimum of two years’ supervisory experience

 

Physical Demands:

  • Ability to be physically mobile (stand, walk, reach, climb, bend) and also the ability to sit for prolonged periods throughout the day/work hours
  • Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type
  • Must be able to occasionally lift and/or move up to 25 pounds
  • Occasional driving to client appointments, offsite meetings, conferences, etc.

Other Requirements:

  • Valid driver’s license
  • Recent physical exam and TB screen within 12 months
