Description

Position Summary:

The AFC provider may employ or independently contract with community health workers. Community Health Worker Consultant is a contract public health worker position with at least one year of experience working with elders and adults with disabilities who applies their unique understanding of the experience, language, or culture of the populations they serve in order to carry out their responsibilities in AFC-qualified settings. In addition, this person is responsible for identifying and accessing additional resources to support and to assist clients and their families in his/her caseload, and for maintaining direct communication with the AFC Program Director.

 

Duties and Responsibilities:

The responsibilities of the Community Health Worker include, but are not limited to:

  • Participating in the development, implementation, and ongoing review of the AFC plan of care;
  • Completing Community Health Worker progress note corresponding with each on-site visit and encounter with members
  • Providing to the member and caregiver culturally appropriate health education, information, support, and counseling.
  • Selecting, training, evaluating, and supervising AFC caregivers in conjunction with the registered nurse
  • Conducting on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse) for Level I, and monthly for Level II;
  • Completing a care-management progress note corresponding with each on-site visit or encounter and upon significant change
  • Periodically reviewing AFC caregiver logs
  • Assisting with obtaining information and accessing other healthcare and community services
  • Reviewing and documenting the fire and safety procedures for the qualified setting
  • Participating in discharge planning and implementation of discharge
  • Reviewing annually the suitability of the qualified setting.

Mandated Reporter Statute: An individual in this position is considered a “Mandated Reporter” when on, or off duty according MA Statutes.
 

Qualifications & Skills:

  • Bilingual Required (Spanish)
  • Excellent written and oral communication skills
  • Excellent organizational skills
  • Able to work in group collaboration, as well as independently
  • Self-motivated
  • Resourceful
  • Able to meet deadlines
  • Able to multi-task

Education & Experience:

  • At least one year of experience working with elders or adults with disabilities or a combination of the two.

 

Physical Demands:

  • Ability to be physically mobile (stand, walk, reach, climb, bend) and also the ability to sit for prolonged periods throughout the day/work hours
  • Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type
  • Must be able to occasionally lift and/or move up to 25 pounds
  • Occasional driving to client appointments, offsite meetings, conferences, etc.

Other Requirements:

  • Valid driver’s license
  • Recent physical exam and TB screen within 12 months
